Record of Achievement Guide & Info
The Record of Achievement allows you to upload a branded template PDF file which includes Fillable Form Fields, these then can include the following field variables in the template, these will then be populated when the ROA is created from an apprentice.
In order to edit the fillable form fields on a PDF template, you will need to use some proprietary software such as Adobe Acrobat DC. (Note this is not the free Adobe reader).

There are also free online PDF editing tools that you can use, or some that offer free trials before charging you.
We have located and tested this Free Tool called Sejda https://www.sejda.com/pdf-forms which works, it is basic and it will allow 3 free document edits, before it locks you out for an hour.
Unfortunately we have found since a recent update to https://www.pdfescape.com/ the form fields are no longer working, if you use this to create your fillable form fields I am afraid you will need to find an alternative.

ROA Options
Component level ROA - To create a component specific ROA, you upload these directly under the Assessment Component.
Overall Certification Grade ROA - In order to create an overall certification ROA you upload the PDF against the Assessment Schema
Steps to creating a RoA template
Create a Word document with your chosen design, logos and layout etc.
Convert this document into a PDF by saving as a .pdf file when you save it in Word.
Either sign up to a free trial of some PDF Fillable Form Field product or use any existing software you have for this purpose such as Adobe Acrobat DC.
Convert the fields to Fillable Form Fields and enter the field names e.g. apprentice_name into the relevant field.
When you have finished the document would look something like this:-

The information can be found via the tooltip on the Entity Management → Assessment Component or there is a similar tooltip against the Assessment Schema.
